This pattern is available for sizes L to 5X.
This pattern is knit in the round on circular needles from the bottom up. It is joined with the sleeves and then uses raglan decreases up to the neck.
Many women tried it on at the book launch party and it looked fabulous on each of them, I kid you not.

Here is a supplement with original sleeve instructions and tips for modifying neckline to fit. The pattern was edited a lot for space for the book.

Also note: the length of the sweater should be high hip with 3/4 sleeves. Change the length of sleeves and body to 14” for most women. (Designer is 6’-0” tall.)
