Black Embroidery Thread

Bonded Felt (optional, music notes)
Hot Glue (optional)
Safety eyes are not recommended for children under 5 so if you intend for item to be a toy instead of decoration I would choose to sew on eyes with embroidery thread.

Abbreviations: 

 BLO = Back Loop Only 

CH  = Chain

HDC = Half-Double Crochet

INC = Increase (Increase = 2 STS in same ST)

MR= Magic Ring

SC  = Single Crochet 

SL ST = Slip Stitch
