Sleeve pattern is wrong--according to the chart, the cast on (and subsequent stitch counts after increases) is 2 stitches short.

Also, the row count is off. If you follow the pattern and are knitting to gauge (2.75 stitches - 3.75 rows per inch), you wind up with a sleeve length that’s 23.5 inches long for the smallest size--not 21.

Cuff = 11 rows
Chart = 24 rows
Increases past chart = 39 rows
top shaping = 14 rows

By the time you get to the stitch count you should have before starting to shape the top, you’ve got 19.75 inches with 3.75 inches of shaping to go!
