Used a Crochet Provisional Cast-on. (The pattern does not include any instruction for the provisional cast-on.)

Holding Cable Needle (a DPN) to the back.

Mod: Ended up knitting 7 full repeats plus 14 rows, as many other Ravelers have done. This results in a better fitting cowl for my purposes. This also used 4 skeins of Capra DK, rather than needing a partial 5th skein.

I found this video and this video helpful for grafting the two ends of the tube. The pattern does not include any instructions for the grafting. I have done kitchener hundreds of times, but generally for socks.
